WHAT DOES IT COVER?
Damage and Theft
Applies to all mobile phones owned by the primary account holder
No pre-registration required
Includes cracked screens, liquid spills, burglary and robbery
Pays up to $500 per claim minus a $50 deductible
Limit of 2 claims per consecutive 12-month period.
Coverage for damage begins the first day of the calendar month following the benefit effective date; coverage for theft begins on the benefit effective date.
Damages that occur during the waiting period are not covered
HOW DO I FILE A CLAIM?*

Contact your Program Sponsor, who will verify your eligibility and send you an online claim form to complete.
Go to the online claim form within 60 days of the loss or damage of your covered device.
Enter all the required information on the electronic claim form.
Provide all required documentation to the Benefit Administrator within sixty (60) days from the date of damage or theft of your eligible cell phone or your claim may be denied.
Once all required documentation for your claim has been received, your claim will be reviewed. Upon approval, you should receive reimbursement within 5 to 10 business days.
